    Essential to achieving a sustainable recovery is access to an appropriate aftercare addiction programme after you have completed our rehab programme. Appropriate aftercare ensures a smooth transition between treatment and your daily life afterwards and reduces the chances of a relapse.

    During the addiction assessment

    It is important to start organising substance abuse aftercare as early as possible, which is why we will be discussing yours during the assessment interview. What care may be needed following our treatment and how can we arrange this? Shortly after the assessment, one of our aftercare coordinators will call you on this matter. If necessary, we can already set things in motion.

    During the rehab programme

    During your stay in our rehab centre, our aftercare coordinators will continue to guide and support you in organising aftercare. They do this  by establishing and maintaining contact with our network partners, in consultation with your therapists. We strive to have you enter an aftercare programme for substance abuse immediately after your admission.

    National network partners

    To ensure you receive the best possible aftercare, Connection SGGZ makes use of a national network of various partners:

    • safehouses
    • outpatient care providers and addiction counseling services
    • mental health care institutions (GGZ) for treatment of underlying causes

    Follow-up process at CACN

    If you live within travelling distance of Eindhoven, it is possible to enter a follow-up programme at our chain partner CACN after inpatient rehab. Here, you can attend a part-time programme of two days a week, with both group and individual aftercare therapy.

    Safehouse

    A safehouse is a refuge where a person can stay after they have been treated for addiction, in case they need intensive supervision. If needed, you can enter a safehouse programme immediately after your stay at the clinic and follow a programme aimed at reintegration.

    Outpatient aftercare treatment

    This treatment consists of weekly one-on-one sessions with a psychologist and focuses on the addiction spectrum and on root causes. Discussions are sometimes combined with group sessions (one per week for example) and additional online contact (eHealth).

    Outpatient home counselling

    During an outpatient home-based support programme, you will receive individual support in various areas of life, such as living, working and finances. The duration of this route is based on your needs. You will also be supported in attending AA/NA meetings. You will receive this care from your own home, with in some cases periodic group contact (information, themes, skills training, etc.).

    Of course, you will receive extensive information, explanations and guidance during the addiction assessment and afterwards to build an aftercare programme that is tailored to your specific needs.
